Complaint Investigation Report
The investigation revealed the following:
In regards to the allegation " Staff hit resident.” It is alleged that a client was hit on the head with a closed fist by a staff member. On 02/11/2026, the client reported to the administrator that the incident allegedly happened on 02/09/2026, in the presence of another staff who did not say anything. All (5) staff interviewed denied the allegation and stated they had never hit C1 nor had seen any staff hit C1 or any clients. Interview with S1 revealed that C1 called their family member repeatedly on Monday afternoon, February 9, 2026, to ask them to bring dinner for them, but they never answered, which made C1 upset. Staff stated that C1 took their evening medication, went to sleep and did not show any aggression. S1 stated there was no hitting. S1 also stated that the Administrator called S1 on 02/11/2026 and was put on leave until investigation was completed. Staff indicated that West Covina Police arrived on 02/13/2026 to investigate the alleged incident but there were no findings. On the same day, paramedics came and checked C1's vital signs and did a body check, but they didn't bring C1 to the hospital. C1's interview revealed that they lied and C1 recanted their story. C1 stated that S1 did not hit them. Additionally, C1 stated that C1 just apologized to S1 earlier and felt bad about lying. Review of C1's most recent Individual Program Plan (IPP) indicates that C1 has a history of attention seeking and fabrication. Per Quality Assurance representative, the Regional Center is currently reviewing the claim and a report will be sent once completed.
Based on statements and interviews conducted with client and staff as well as reviewed files and documentation, there was not enough supportive evidence to corroborate the allegation.
Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is UNSUBSTANTIATED.
