Facility Evaluation Report

Licensing Program Analyst (LPA) Cynthia Chan conducted the required annual inspection. LPA arrived unannounced and met with Administrator, Victor Orunesajo. The reason for the visit was explained. The facility is licensed for (5) ambulatory adults ages 18 -59. There are currently 3 clients residing at the facility and receiving services through the San Gabriel/Pomona Regional Center.

LPA inspected the facility using the Compliance and Regulatory Enforcement (CARE) tools.

The facility consists of 4 client bedrooms, 2 bathrooms, living room, dining room, and kitchen. There is a laundry room and and a garage. There are extra linens and hygiene supplies. Facility has interconnected smoke detectors and a carbon monoxide detector located in the hallway. Knives, cleaning solutions, and disinfectants are locked. The hot water temperature was measured between the required range of 105-120 degrees F. Staff are providing care and supervision to meet the clients' needs. Staff are continuing to follow their infection control plan and procedures while caring for clients. There are sufficient food supplies of 2 day perishable and a week of non-perishable items. There is sufficient staffing and facility has one overnight staff awake to supervise clients. Staff are fingerprint cleared and associated to the facility. LPA reviewed records for 3 staff and 3 Clients. Administrator's (Victor Orunesajo) certificate expires on 6/11/25. Staff have current CPR & First Aid certificates. Staff do not use any manual restraints on clients. Medications are centrally stored and locked. All 3 clients' medications are being administered as prescribed. The facility has the updated emergency and disaster plan and conducting monthly disaster drills.

No deficiencies were observed today. An exit interview was held and a copy of this report was given to the administrator.
