The inspector’s account

On 07/02/26, Licensing Program Analyst (LPA) Regina Cloyd conducted an unannounced Required – 1 Year Inspection to the above-named facility and met with Administrator Leilani Le. LPA explained the purpose of the visit. The facility is licensed to serve six ambulatory clients age range 18-59.

The facility is a two-story home in a residential neighborhood. The facility consists of four (4) bedrooms, two (2) bathrooms, kitchen, dining room, living room, sitting room, staff office, detached garage, and an outdoor area. On the first floor are two (2) bedrooms, bathroom, kitchen, dining room, and sitting room. On the second floor are two (2) bedrooms, bathroom, living room, and staff office.

Outside grounds were toured and no bodies of water were observed. Walkways around the home were clear of hazards. There are no weapons on the premises.

Four client’s bedrooms were checked. Mattresses were in good condition, adequate lighting, plenty of dresser and closet space observed. Walls and floors were clean and in good condition. Comforters, bed linen, bath towels and mattress protectors were adequately stocked. Bathroom toilets and water faucets worked properly. Adequate lighting and toiletries in stock. LPA Cloyd tested hot water temperature, and it measured at 108 degrees Fahrenheit. This facility provides clients with hygiene products such as nonmedicated soap, toilet paper, toothbrush, toothpaste, and comb.

LPA observed supplies of nonperishable foods for a minimum of one week and fresh perishable foods for a minimum of two days in the kitchen. The facility has a chef who prepares and delivers three meals to the clients. Continue to LIC809-C.
