The inspector’s account

On 07/30/25, Licensing Program Analyst (LPA) Sparkle Day conducted an unannounced Continued Annual Required visit using the CARE Inspection Tool. On 6/16/2025 LPA Yolanda Rosser Conducted the Annual Required visit, However due to time restraints LPA Rosser did not complete the Annual Inspection. LPA Day met with Lelani Le, Administrator and the purpose of today’s visit was explained. The facility is licensed to operate for (6) ambulatory adults ages 18 through 59. Currently the home has (4) clients.

During the visit On 6/16/2025 LPA Rosser completed the following: Inspected the facility inside and out including. This is a two story family home located in a residential neighborhood. The property consists of the following: DOWNSTAIRS 2 client bedrooms, 1 common bathroom, kitchen with washer and dryer, dining area and living room area. UPSTAIRS-_2 Client bedrooms, activity area, 1 common bathroom, Med Room. There is a detached garage that's used for storage and extra supplies and outside lounging area with umbrella.

LPA Rosser conducted a records review of (5) client records. LPA reviewed the facility disaster plan. All client records were complete. The facility disaster plan was current and in compliance with Title 22 at the time of visit. Clients attend a Day Program. LPA conducted MAR review via Best Notes.

During today visit LPA Day conducted a file review of (5) staff records, LPA reviewed (4) Client Medication Administration Records and did not observe any discrepancies at the time of visit.

At 11:00am LPA and Administrator Le toured the inside and outside of the facility. All client rooms were checked. Mattresses and box springs were in good condition, adequate lighting was observed, plenty of dresser and closet space was observed. Walls and floors were clean and in good repair. Bed linens, comforters and bath towels were adequately stocked at the time of visit. Bathrooms were found to be within
