Facility Evaluation Report
On 06/16/25, Licensing Program Analyst (LPA) Yolanda Rosser conducted an unannounced annual required visit using the CARE Inspection Tool. LPA met with the Administrator Leilani Le and the purpose of today’s visit was explained. The facility is a short term crisis program licensed to operate for (6) ambulatory adults ages 18 through 59. None of the clients have Restricted Health Care Conditions, and none are utilizing postural supports or protective devices.
The facility is a two story family home located in a residential neighborhood. The property consists of the following: DOWNSTAIRS 2 client bedrooms, 1 common bathroom, kitchen with washer and dryer, dining area and living room area. UPSTAIRS-_2 Client bedrooms, activity area, 1 common bathroom, Med Room. There is a detached garage that's used for storage and extra supplies and outside lounging area with umbrella.
LPA conducted a records review of (5) client records. LPA reviewed the facility disaster plan. All client records were complete. The facility disaster plan was current and in compliance with Title 22 at the time of visit. Clients attend a Day Program. LPA conducted MAR review via Best Notes.
Due to time restraints LPA will return at a later date to complete inspection.
