Facility Evaluation Report

Licensing Program Analyst (LPA) Zabel Chochian conducted a Case Management visit at the above facility at approximately 1:30pm to verify the relocation of Resident #1 (R1), Resident #2 (R2) and Resident #3 (R3) to this facility and to confirm their health and safety following the transition. LPA met with staff and explained the purpose for the visit. Staff contacted Administrator and LPA explained the reason for the visit.

At approximately 1:30pm, LPA Chochian and staff conducted a tour of the physical plant to ensure compliance with Title 22 regulations, which included a tour of all resident rooms. R1, was observed on the couch resting, R2 and R3 were observed in their respectful room. LPA met with the residents and attempted to interview them. Residents observed to be well groomed, and happy during LPA's visit.

No immediate health or safety concerns were observed during the visit. Exit interview conducted. A copy of the report was provided.
