Facility Evaluation Report

Licensing Program Analyst (LPA) Martha Arroyo arrived at the facility unannounced to conduct the required annual visit today at 11:55am. Upon arrival, the LPA was greeted by the Administrator, Anait Domio, and the reason for the visit was explained. The four (4) residents were out in the community during the inspection. Entrance interview conducted.

Beginning at 12:05pm, the LPA along with the Administrator toured the physical plant areas inside and outside to ensure there are no health and safety hazards, and facility is in compliance with Title 22 Regulations. The following was observed:

Kitchen: The LPA inspected the kitchen/food service area at approximately 12:17pm. Knives and sharps were observed in a locked drawer inaccessible at the time of the visit. Cleaning supplies were observed locked and inaccessible under the kitchen sink. Kitchen appliances were in operable condition. The facility has a sufficient supply of perishables and non-perishable food. Refrigerator and food pantry were checked for proper labels and expiration dates. Dates were clearly marked. At 12:23pm, the hot water temperature was measured in the kitchen sink, and it measured at 106.7 degrees Fahrenheit.
