Facility Evaluation Report

Licensing Program Analyst (LPA) Sandra Urena conducted an unannounced Case Management-Other inspection at the facility today as the licensee has informed the LPA that the facility does not have any residents and that the facility is officially closed. The LPA contacted and spoke on the phone with Savvoy Toney Director of Residential Facilities (DRF) to inform them of the visit. The reason for today's inspection is to confirm the closure of the licensed facility. When the LPA arrived there was no one at home. The LPA contacted the DRF who stated that they had already returned the keys to the landlord. The DRF stated that they would send someone to open the property for the LPA. The LPA met with Rafael Lopez, Facilities Manager (FM) at approximately 11:35 a.m.

On 08/15/2025, LPA Urena received information via email from Savvoy Toney Director of Residential Facilities that the facility was officially closed. Three residents have been relocated to new facilities, and they are officially no longer operating a licensed facility. Prior to today's inspection, the LPA confirmed the relocation of the three (3) residents.

LPA Urena toured the physical plant and looked through the windows, given that the licensee no longer had access to the facility. The LPA observed the home to be empty and found no evidence the home is operating as a licensed facility. Based on observation and interview, the facility is no longer operating as a licensed facility. As soon as the surrendered license is received, the facility will officially be closed. The LPA explained the process to the FM, and the FM understood the process. They stated they will be mailing the surrendered license and final closure letter to licensing via US postal mail to the Woodland Hills North Regional Office, attention to LPA Urena.

Closure of the facility is confirmed as of this visit. Exit interview conducted. A copy of the report was issued.
