Facility Evaluation Report
Licensing Program Analyst (LPA) Zabel Chochian conducted a Case Management visit at the above facility at approximately 2:45pm to verify the relocation of Resident #1 (R1) to this facility and to confirm their health and safety following the transition. LPA met with staff Abriiana Thomas. Staff contacted the Executive Director (ED) Yehuda Cohen. LPA spoke with ED and explained the purpose for the visit.
At approximately 3pm, LPA Chochian and staff conducted a tour of the physical plant to ensure compliance with Title 22 regulations, which included a tour of R1’s room. During the tour, R1 was observed to be in the activity room. The LPA met with R1 privately in their respectful room. R1 appeared groomed, alert and happy at the time of LPA's visit.
No immediate health or safety concerns were observed during the visit.
