Licensing Program Analyst (LPA) Christine Yee conducted an unannounced case management visit due to deficiencies observed on a visit to the facility today. The reason for today's visit was provided.
On today's visit the following was observed:
Staff, Kumissay Ossorio, was observed providing care to the client's during today's visit. Per discussion with staff, they started working at the facility on 2/1/26.
Deficiencies cited under California Code of Regulations, Title 22, Division 6, Chapter 1 ******** Immediate civil penalties were assessed.
Exit interview was conducted, APPEALS RIGHTS discussed and a copy was provided.
