Facility Evaluation Report
Licensing Program Analyst (LPA) Sandra Urena conducted an unannounced required annual visit. LPA Urena met with Program Director Michael Smith, and explained the reason for the visit. At the time of the visit most participants were in attendance at the facility due to inclement weather. The program is a community based program.
LPA Urena and the Program Director toured the physical plant areas inside and outside ensuring that there are no health and safety hazards, and that the facility is in compliance with Title 22 Regulations.
COMMON AREAS: The LPA observed seven activity rooms, one computer room, and one Refresh room. All rooms were equipped with appropriate furniture, and were observed to be well lighted, clean and in good condition. At the time of the visit, common seating area and office furniture were observed to be in good condition. The LPA observed the required postings throughout the facility. Fire extinguishers were observed to be fully charged and last serviced on 05/08/2025. The smoke detector and carbon monoxide detectors were tested and operational at the time of the visit. The facility is fully gated. LPA observed two gates to be unlocked at the time of the visit. LPA did not observe any obstructions to exits at this time. The facility maintained a comfortable indoor temperature of 71 degrees.
There is a storage/pantry room that is permanently locked and inaccessible to participants at the facility. Inside storage/pantry room LPA observed paper plates, and adequate supplies of Personal Protection Equipment (PPE), emergency food, sharp objects, cleaning supplies, blankets, flashlights, emergency backpacks and extra first aid kits.
