Facility Evaluation Report
Licensing Program Analyst (LPA) Angela Barutyan arrived at the facility unannounced to conduct an annual required inspection at 11:55AM. LPA arrived at the facility and rang the bell and knocked several times and there was no answer. Administrator Danny Tapia was contacted via telephone call and stated that a representative will arrive to assist the LPA with the inspection. Per the Administrator, the facility’s last client left in December of 2024, and was relocated by the North Los Angeles County Regional Center to another facility. The facility has been vacant since December of 2024. LPA met with Administrator Barbara Pacheco at 12:38PM. Entrance interview conducted.
The LPA and the Administrator toured the physical plant areas inside and outside to ensure there are no health and safety hazards and facility is in compliance with Title 22 Regulations.
KITCHEN/LAUNDRY: Knives and cleaning supplies are stored inaccessible. Kitchen appliances were in operable condition. At the time of the visit, the facility did not have clients. Consequently, the facility did not have any perishable and non-perishable food. The Administrator was advised to inform the CCL Department prior to admitting clients, so that a case management visit-other is conducted to ensure that the facility is ready to accept new clients. Washer and dryer are located in the kitchen area. Sharp objects are stored in a locked cabinet next to the washer and dryer. At 12:55PM, LPA observed the cabinet containing sharps to have a faulty lock as it had difficulty locking and the locked cabinet above unable to be unlocked. Administrator stated that the locks on the cabinets will be repaired.
BEDROOMS: There are three (3) single-client bedrooms which were furnished appropriately with clean linens, appropriate furnishings and sufficient lighting. The exterior exit door in Bedroom #1 had a functional auditory exit alarm. Report Continued on LIC 809-C.
