Facility Evaluation Report
Licensing Program Analyst (LPA) Quoc Huynh arrived at the facility unannounced to conduct a required annual visit at 10:10AM. The LPA met with the Program Director (PD) Jose Chavez and informed them of the reason for the visit. Entrance interview conducted.
Beginning at 11:23AM, the LPA and PD toured the physical plant areas inside and outside to ensure there are no health and safety hazards, and facility is in compliance with Title 22 Regulations. The following was observed:
COMMON AREAS: At the time of the visit, all common areas were furnished appropriately, had sufficient lighting, and no hazards were observed. Required postings were observed throughout the facility. The common areas included the main activity room, pantry, front and back computer room, administrative offices, conference room/staff break room, client restrooms, staff restrooms, and kitchen. The pantry stored emergency food and water that was also utilized as a community activity food drive and is frequently replenished. The kitchen was utilized as part of the program’s activity, did not store any hazardous items, and meals were not prepared on site. The client restroom’s hot water was tested and measured at 101.3 degrees F and the LPA advised to increase the water temperature to maintain compliance.
