Staff did not ensure resident was dressed in residents personal clothing.
On the allegation that the facility staff did not ensure that the residents were wearing their personal clothes, it was observed that the resident was wearing what appeared to be men’s underwear. The resident has been observed wearing the same underwear at least four different times. The observation was brought up to the facility staff Marsha who purchased the underwear, and Marsha replied, “It’s o.k”. On 05/08/2026, the LPA was able to view the piece of undergarment; however, the tag of the underwear does not identify the gray and striped “bikini shaped” undergarment as men’s or women’s undergarment.
Although the allegation may have happened or is valid, based on observation, there is not sufficient evidence to prove that the underwear is men’s underwear, therefore the allegation is deemed Unsubstantiated at this time.
Staff are not intervening when another resident is providing bathing, hygiene to another resident in care.
On the allegation that facility staff are not preventing residents assisting other residents with their hygiene and bathing routines in the morning. It is alleged that when staff questioned the residents(R2) about who was bathing the resident (R1) resident (R2) pointed to themselves as the one giving a bath to the other resident (R1). The LPA Urena interviewed facility staff about the residents’ bathing schedule and routine. The staff reported that showers for R1, staff assist R1 with showers 3 times a week, usually at 6:00 a.m. LPA Urena was unable to interview R1 and R2. The LPA asked if the staff have witnessed residents assisting each other with bathing or hygiene, the staff denied witnessing such activities.
Although the allegation may have happened or is valid, based on the interviews, there is not sufficient evidence to prove the alleged violation(s) did or did not occur, therefore the allegation is deemed Unsubstantiated at this time.
