Complaint Investigation Report
The tour of the physical plant revealed that the facility is being altered by painting the walls, changing light fixtures in the bedrooms, kitchen cabinets were replaced and the gas line for the stove was moved from one side of the kitchen to the opposite side of the kitchen wall. The Administrator did not inform the Community Care Licensing Division nor the LPA in an official letter of the extensive alterations being made to the facility, prior to the start of the alterations work done.
Licensee did not obtain appropriate permits prior to construction/alterations to the facility.
The LPA interviewed the Administrator and asked if city permits were obtained prior to the alterations being made to the facility, and the Administrator stated that no permits were obtained.
Based on observations, and interviews, the Administrator did not notify the licensing agency of the proposed changes, prior to construction/alterations to the facility; nor did the Administrator obtained city permits prior to the initiation of the construction, therefore the allegations are deemed Substantiated at this time.
Pursuant to Title 22, California Code of Regulations, the following deficiencies were cited (refer to LIC809-D).
Exit interview conducted. A copy of the report was issued and Appeal Rights were provided.
