Facility Evaluation Report

LPA Urena along with with Quality Assurance, Lisseth Carrillo , Community Services Specialist - QA/CPP

Community Services LDC-MFP | North LA County Regional Center, conducted a case management other visit to the facility, to ensure that the facility meets regulations and it has been cleared by the Los Angeles Fire Department and the Building and Safety department.

The LPA and QA/CPP along with the Administrator, toured the physical plant areas inside and outside to ensure there are no health and safety hazards and that facility is in compliance with Title 22 Regulations.

The facility was observed to be clean, set up and ready for the residents to return to the facility after LPA Urena speaks with the LAFD Inspector.

Exit interview was conducted. A copy of the report was issued.
