Facility Evaluation Report

Licensing Program Analyst (LPA) Martha Arroyo conducted an unannounced case management visit to discuss the opportunity for the facility to be referred to the Technical Support Program (TSP). Upon arrival, the LPA met with Licensee Representative, Alexander Otiashvili and at this time the reason for the visit was explained. Entrance interview conducted.

During today’s visit, a physical plant tour was conducted to ensure there are no health and safety concerns. In addition, the LPA encouraged the Licensee Representative to consider participating in the TSP. This is a non-enforcement program offered by the Community Care Licensing Division (CCLD) that provides services and resources to assist providers in meeting licensing requirements. The TSP is designed to help facilities achieve and sustain compliance through training, support, and access to relevant tools. The LPA provided the Licensee Representative with the following contact information for TSP services. TSP email address TechnicalSupportProgram@dss.ca.gov and their phone number (916) 654-1549. The Licensee Representative agreed to contact TSP for further information and assistance.

Additionally, the LPA also conducted a follow-up in reference to deficiencies that were cited during the annual continuation visit that occurred on 07/10/2025. The LPA and Licensee Representative reviewed previous citations and discussed plan of corrections. The Licensee Representative stated that they will work on the plan of corrections and agreed to email copies of pertinent documents to CCL no later than Monday 07/21/2025. Documents requested include but are not limited to; plan of corrections from 07/10/2025, resident admissions agreement and facility’s bank account information.
