Facility Evaluation Report

Licensing Program Analyst (LPA) Christine Yee conducted an unannounced case management visit to check on the health and safety of the residents in care due to the pending relocation of the Clients once the Regional Center vendorization process is completed. LPA Yee met with Anait Domio, Administrator and spoke with Marine Hakobyan via telephone regarding the reason for today's visit

Present at the facility during the visit was Anait Domio, Administrator, Ofelya Zakaryan, Staff and Client #1. Three of the clients were away at day program. Client #1 does not attend day program and is in the process of looking for employment. Per visual observation of Client #1 they looked well and per conversation with Client #1, they are excited and looking forward to moving to the new home.

Per visual observation of the physical plant, the home looked clean and well maintained. No visually obvious concerns were observed during the visit.

During today's visit, the Administrator was reminded to let LPA Yee know when the vendorization process is completed and when the relocation is scheduled and when the move is completed. Once the relocation is completed, LPA Yee will return to conduct a closure visit.
