Licensing Program Analysts (LPAs) Brian Balisi and Martha Arroyo conducted a Case Management - Deficiencies visit in conjunction with a complaint visit (Complaint Control # 29-AS-20251013092407). The purpose of the visit is to issue citations for deficiencies observed during the initial complaint investigation.

During the course of the investigation, it was revealed that Staff #1 (S1), an employee of Age Well Assisted Living Inc., collected the Electronic Benefits Transfer (EBT) card from Resident #1 (R1) several months ago. R1 had provided S1 with a list of items to purchase on their behalf. However, as of today, S1 has not provided the requested items, and the EBT card balance is currently $0.

Additionally, a review of the facility’s Affidavit Regarding Client/Resident Cash Resources (LIC 400) indicates that the facility is not authorized to handle any cash resources belonging to residents. However, staff assisted the resident with safeguarding their funds. As a result, the facility is required to maintain Surety Bond Insurance. Yet, the facility did not obtain this insurance until 10/13/2025, after being informed of the necessary requirement to handle resident’s monies.

The following deficiencies were cited from the California Code of Regulations, Title 22 and California Health & Safety Code. (See LIC 809-D). The Administrator was informed that failure to correct deficiencies may result in future civil penalties.
