The inspector’s account

Licensing Program Analyst (LPA) Zabel Chochian conducted a Case Management - Deficiencies. LPA met with individual/staff Kristine Mesropyan. Staff attempted to contact Administrator Lala Soghomonyan.

At approximately 11am, LPA toured the facility with staff and met with three residents. All three residents interviewed expressed being satisfied with the service and food. LPA observed a sufficient supply of perishable and non-perishable food properly stored during today's visit. At approximately 12:15pm, LPA spoke with Administrator Lala Soghomonyan and the reason for the visit was discussed. Administrator stated that she is at least 3hrs away from the facility and is unable to come to the facility. LPA requested that Administrator contact Owner/operator Sarkis Dovlatyan to inform him of LPA's visit and see if he would be able to come to the facility. At approximately 12:40pm, LPA received a call back from Administrator stating that she was unable to reach Mr. Sarkis Dovlatyan. At approximately 1:15pm, LPA and Licensing Program Manager Desaree Perera spoke with Mr. Sarkis Dovlatyan and explained the reason for the visit today.

California Department of Social Services criminal clearance record database was reviewed and learned that staff #1 is not fingerprint cleared to work and has been working at the facility without a criminal record clearance. Interviews and records review confirmed that staff #1 has been working at the facility since 08/31/2025. During todays visit, interview and records review revealed that staff #2, hired since 9/6/2025 completed a request for live-scan however facility did not receive a criminal record clearance for the staff prior to staff being present at the facility as a caregiver since 9/6/2025-9/10/2025. Administrator came to the facility with reliever staff to cover shift and removed staff #2 from the facility during today's visit pending criminal record clearance. An updated LIC 500 showing 24/7 staff coverage schedule was obtained.

Following deficiency was cited from the Title 22 California Code of Regulations. Civil penalty issued today for facility staff not having fingerprint clearance. Exit interview held. Copy of report and appeal rights provided.
