Facility Evaluation Report
Licensing Program Analyst (LPA) Martha Arroyo conducted a Case Management - Deficiencies visit to address deficiencies observed during an unrelated complaint investigation. Entrance interview conducted.
During today’s visit, interviews revealed that Staff #1 (S1) has been employed at the facility for approximately one (1) month. The LPA reviewed the facility's Guardian roster and discovered that S1 does not have fingerprint background clearance and is not associated with the facility. The Licensee was informed that S1 must obtain fingerprint clearance and be properly associated with the facility prior to beginning employment. Relieving staff that is cleared arrived to provide the care and supervision of all residents, and S1 left the premises. Additionally, the LPA attempted to review Resident #1’s (R1’s) file; however, it was not available or accessible at the facility during today’s visit.
The following deficiency was observed (See LIC 809-D) and cited from the California Code of Regulations, Title 22 and/or California Health and Safety Code. An immediate $500 civil penalty issued today for facility staff not having fingerprint clearance.
