Facility Evaluation Report
Licensing Program Analyst (LPA) Brian Balisi conducted a Case Management - Deficiencies visit in conjunction with the complaint visit (Complaint Control #29-AS-20250611092348). Reason for this report is to issue deficiency observed during the complaint investigation which were unrelated to the complaint allegation. During today’s visit, LPA met with Licensee Sarkis Dovlatyan and explained the reason for the visit.
During records review, LPA did not observe a full resident file completed for Resident #1 (R1).
The following deficiencies were observed (See LIC 809-D.) and cited from the California Code of Regulations, Title 22 and California Health and Safety Code. Failure to correct the deficiencies may result in civil penalties.
