Facility Evaluation Report

Licensing Program Analyst (LPA) Brian Balisi conducted an unannounced Case Management – Other inspection at the facility today due to the closure of the licensed facility. Upon arrival LPA attempted to contact Licensee Sarkis Dovlatyan , but they stated they were unavailable for the visit. LPA contacted Acting Administrator Arshalouis Manoukian and explained the reason for the visit. Manoukian arrived at approx 10:30 a.m.

On 12/31/2025, CDSS No. 6124053301 OAH No. was served to the License for the Revocation for the Facility license of Age Well Assisted Living Inc Facility # 197608986.

As of 01/12/2026, the last (2) remaining residents were relocated to The Residences at Royal Bellingham Facility No. # 197608129.

Today’s inspection is to ensure that there are no longer any elements of care or supervision of a licensed facility being provided at this location and to officially close the facility. At 10:35 a.m., LPA and Manoukian conducted a brief tour of the home. During the inspection, the LPA found no evidence to support that the home is being operated as a licensed facility. The license was forfeited at the time of the visit. Exit interview conducted and a copy of the report was provided.
