Facility Evaluation Report
Licensing Program Analysts (LPA) Brian Balisi conducted an unannounced Case Management - Other visit to follow up on the relocation of Resident #1 (R1) and Resident #2 (R2). LPA met with Acting Administrator Arshalouis Manoukian and explained the reason for the visit. LPA attempted to contact, Licensee Sarkis Dovlatyan via phone and text message, but was unsuccessful.
During today’s visit, the LPAs conducted a tour of the facility inside and outside to ensure there were no immediate health or safety hazards. All utilities were observed to be on and functioning properly. The LPAs observed the kitchen and food supply and there is a sufficient amount of both perishable and non-perishable foods.
According to Manoukian, R1 and R2 will be relocated to The Residences at Royal Bellingham Facility No. # 197608129. Manoukian stated they have been in constant contact with their Executive Director and will be present when they are relocated. Monoukian will update LPA as soon as they are relocated.
At 11:02 a.m. Licensee text LPA and stated to have Manoukian sign for the report.
No immediate health and safety concerns observed during the visit. Exit interview conducted and copy of report issued.
