Additionally, LPA also conducted interviews with one staff and a resident. The case was referred to Community Care Licensing Division’s (CCLD) Investigations Branch (IB) and assigned to Investigator Michele Salant.

On 09/15/2025, the RO obtained copies of Emergency Medical Service (EMS) reports along with 9-1-1 Audio Recordings and transcripts for the facility dating back to six (6) months. On 09/17/2025, Investigator Salant attempted to obtain reports from the Los Angeles County Medical Examiner’s Office for R1, however, was informed that there were no records for R1. On 09/22/2025, Investigator Salant conducted a visit to the facility and conducted interviews with residents and staff.

On 09/24/2025, at approx. 10:00 a.m., Investigator Salant, attempted to contact Licensee Representative, Sarkis Dovlatyan, but was unsuccessful. Investigator left a voice mail requesting a call back in order to set an interview time. However, at approx. 10:30 p.m., Dovlatyan emailed Investigator Salant with a PDF attachment containing staff member roster, admissions record, medications list for R1 and Resident #2 (R2) along with Hospice records and Physician's report for R2. Dovlatyan also stated he will be out of town (for an undisclosed amount of time) and will not be able to meet Investigator Salant for an interview but stated Administrator Lala Soghomonyan would contact them to schedule a meeting. On 09/30/2025, Investigator Salant interviewed Administrator Lala Soghomonyan at Age Well Assisted Living Facility.

Information gathered during the course of the investigation reflected that R1 was alive and relocated to a different facility. On 10/02/2025, Investigator conducted a visit to Dignity Health and observed R1 alive and well. It was revealed that facility on 06/18/2025, Los Angeles Fire Department (LAFD) arrived and found a deceased individual who was identified as R1 by the staff. However, based on information gathered facility staff disclosed the incorrect information to LAFD. Therefore, the above allegation, “Questionable Death” of R1 is deemed UNFOUNDED at this time.

Deficiencies observed during the course of the investigation that is not related to the complaint will be cited on a separate Case Management – Deficiency.
