The inspector’s account

Licensing Program Analyst (LPA) Zabel Chochian conducted an unannounced case management visit - other to ensure the health and safety of the residents in care. Upon arrival LPA met with staff and explained the reason for the visit. Staff contacted Licensee representative, Sarkis Dovlatyan.

During todays visit, LPA conducted a physical plant tour with staff at approximately 12:05 p.m.. LPA observed a sufficient supply of perishable and non-perishable food properly stored at the time of the visit. LPA observed fresh produce, eggs, sausages, deli meat, chicken and condiments in the fridge; frozen poultry, fish sticks, crab, frozen vegetables and other frozen goods observed in the freezer. LPA also observed dried and canned food items in the storage. Common areas, restroom and resident bedrooms observed clean and odor free. Residents were interviewed from approximately 12:15 and 12:30 p.m. Interviews with residents revealed they have been provided with sufficient meals and snacks since the last inspection visit on 11/12/2025 and residents did not express any concerns at this time.

At approximately 12:45, Mr. Dolatyan called the facility and spoke with LPA. Reason for the visit was explained. Mr. Dovlatyan explained that he is unable to come to the facility and the Administrator Nailah Tatum is currently in class and wont be available until after 5 p.m. LPA contacted the Administrator and left a voicemail message.LPA spoke with Mr. Dovlatyan by telephone again at approximately 1:30 p.m. LPA asked Mr. Dovlatyan if he received the Accusation filed and dated on 11/13/2025. Mr. Dovlatyan stated that he received a great amount of documents few days ago but has not yet reviewed the documents. LPA explained the Accusation document and Mr. Dovlatyan agreed to go over the Accusation over the phone. As LPA began reading the Accusation for License Revocation, Revocation/Forfeiture of Administrator Certificate, and Exclusion Action, Mr. Dovlatyan stated that he does not want to go over this document over the phone. He stated that he will review it with his attorney and will contact the case LPA if he has any questions.
