Facility Evaluation Report

Licensing Program Analysts (LPAs) Brian Balisi and Martha Arroyo conducted a Case Management - Other visit to the facility. The purpose of the visit is to hand deliver an Immediate Exclusion Order to facility administrator Lala Soghomonyan. LPAs arrived at the facility at approx 10:45 a.m. Upon arrival LPA's met Administrator Lala Soghomonyan. Licensee was contacted via telephone and arrived shortly after the visit began. Entrance interview conducted.

It was determined that due to chronic noncompliance resulting in violations of Basic Services, Personal Rights, Neglect, Lack of Care and Supervision, Prohibited Health Conditions, Pre-Admission Appraisal, Maintenance and operation, Reporting Requirements, Personnel Requirements and training, Resident Records, Medical Care Services, Food Services, Physical Plant, Criminal Record Clearance, Operating Requirements, Conduct Inimical, Administrator Qualifications and Financial Abuse, necessitating the Order for Immediate Exclusion from the facility. During today’s visit, an “Order To Individual For Immediate Exclusion From All Facilities” was hand delivered to Lala Soghomonyan for this facility along with (Angel Ray's Senior Care Inc, #197610759, Pegasus Senior Care Inc, #197610789, Dignity Senior Care, Inc #197610582, Leo's Assisted Living II #197610054, ). Lala Soghomonyan was also provided copies of the “Order To Licensee/Facility Of Immediate Exclusion From Facility” for all facilities she is associated to. LPAs witnessed Lala Soghomonyan leave the facility on this date.

The LPAs also hand delivered the “Order To Licensee/Facility Of Immediate Exclusion From Facility” of Lala Soghomonyan to licensee Sarkis Dovlatyan and to Ejmin Gharib, the licensee of the pending application (Age Well Assisted Living #195850516), along with a copy of the “Order To Individual Of Immediate Exclusion From All Facilities” issued to Lala S. During the visit, LPAs discussed with Sarkis that he needs to provide the Department with the documents for a new administrator along with an updated LIC500 within 48 hours.

At approximately 10:50 a.m., the LPAs conducted a physical plant tour to ensure there were no health or safety concerns. No immediate health and safety concerns were observed during the visit.
