Facility Evaluation Report

Licensing Program Analyst (LPA) Brian Balisi conducted an unannounced case management visit - other to ensure the health and safety of the residents in care. Upon arrival LPA met with staff and explained the reason for the visit. Administrator was contacted and stated they would be at facility in approx (1) hour. Licensee was contacted, but stated they were unable to be present at facility.

At approx 10:10 a.m. LPA conducted physical plant and interviewed residents. LPA observed a sufficient supply of perishable and non-perishable food properly stored at this time.LPA Interviewed Administrator over the phone. Interview revealed Resident #2(R2) was admitted into hospital on Sunday afternoon. Administrator stated they will provide incident report by end of the day. At approx 11am LPA followed up with Administrator regarding their ETA and Administrator stated they ran into a delay, but could be at the home in approx 2 hours. LPA contacted Licensee who stated that staff Margarita Dovlatyan will sign in their place.

Interviews with residents revealed they have been provided with sufficient meals and snacks since LPA's last visit on 09/04/2025. In addition, residents did not express any concerns at this time.

During the visit, LPA did not observe any immediate health and safety concerns at this time.
