The inspector’s account

Licensing Program Analyst (LPA) Sandra Urena conducted an unannounced annual inspection visit. The LPA was greeted by staff. The staff contacted the administrator via telephone call and informed of the visit. The Administrator stated they would come to the facility to meet with the LPA. At the time of the visit, no residents were at the facility. All residents were at their respective day programs. The LPA was unable to interview residents.

The LPA and the Administrator toured the physical plant areas inside and outside to ensure there are no health and safety hazards and facility is in compliance with Title 22 Regulations.

KITCHEN: Knives and cleaning supplies are stored inaccessible. Kitchen appliances were in operable condition. The facility has a sufficient supply of perishable and non-perishable food. Knives and sharps are stored in a locked cabinet adjacent to the dining room.

COMMON AREAS: Living and dining room furniture were observed to be in good condition. The smoke detectors were tested and were operational at the time of the visit. The LPA observed required postings throughout the common spaces. The fire extinguisher was charged and serviced on 02/05/2026. A first aid kit and manual were observed in the dining room area.

BEDROOMS: There are three (3) client rooms. two (2) single occupancy and one (1) double occupancy rooms, which were furnished with appropriate linens and required furniture. Adequate lighting in all bedrooms was observed. There is one (1) staff bedroom, and was observed to be locked, and inaccessible to clients in care at the time of the visit. Continues on LIC 809C...
