Facility Evaluation Report
Licensing Program Analyst(LPA) Christine Yee conducted an unannounced required Annual Inspection using the entire CARE INSPECTION TOOL. Upon arrival at the facility, it was observed that there was no one present at the home. Contact was made via telephone with Liana Aidnova, Licensee and Raelynn Urbanski, Staff arrived at 10:47am to let LPA Yee entry into the home. Liana Aidinova, Licensee arrived at 11:27am to conduct the visit. The reason for today's visit was explained.
The facility is a single storey family home consisting of a living room, dining room, kitchen, 4 bedrooms of which one is used by live-in staff and 3 full bathrooms. The facility is fire cleared for 4 AMBULATORY clients.
On today's visit all 12 domains of the Care Inspection Tool was reviewed, files for 4 clients and 3 staff were reviewed and a tour of the entire home, inside and outside was conducted. The following was observed on today's visit:
the living room, dining room, kitchen were all appropriately furnished and equipped for its designated use.
Bedroom #1 and Bedroom #2 are private rooms and bedroom #3 is a shared room. All the bedrooms are furnished with a bed, a night stand, a chair, a lamp and a dresser for each client and a shared built in closet. Located inside bedroom #3 is a private bathroom equipped with a shower, a toilet and a single sink. A slip resistant mat was observed. The water temperature was tested and read 113.2 degrees
