Facility Evaluation Report
Licensing Program Analyst (LPA) Quoc Huynh conducted an unannounced Case Management visit to inspect the facility's completed and permitted building addition. The LPA arrived at 2:44PM and met with Administrator Heidi Schofield.
On 12/12/2024, the Department received notification of pending construction of an attached garage conversion into a client bedroom which included a full restroom. The building permit was approved as a Junior Additional Dwelling Unit (JADU) that required a separate address from the licensed facility address.
On 07/30/2025, the Department received notification of completed construction. On 04/07/2026, the approved fire clearance was granted for five (5) ambulatory at 12655 Emelita Street and one (1) ambulatory at 12653 Emelita Street.
During today's visit, the LPA conducted a physical plant tour of both addresses at 2:46PM and no immediate concerns were observed. The JADU was in compliance with regulations at this time and had all required furniture, sufficient lighting, and a direct exit to the outside. The JADU contained a kitchenette, attached restroom with hot water that measured at 107.3 degrees F, and a fully charged fire extinguisher that was purchased on 04/04/2026. The hardwired smoke detector was tested at 2:56PM and operational with the main house.
No deficiency cited. Exit interview conducted. A copy of the report was reviewed and provided.
