Facility Evaluation Report

Licensing Program Analyst (LPA) Quoc Huynh conducted an Annual Continuation visit alongside Department of Developmental Services (DDS) Nohely Macias and North Los Angeles Regional Center (NLARC) Nurse Consultant Jane Mandtana. The LPA arrived at 9:40AM and met with Administrator Laurie Hernandez and explained the reason for the visit. Entrance interview conducted.

Beginning at 9:44AM, the LPA, DDS, NLARC, and Staff conducted a medication review. The LPA reviewed medications for one (1) resident. Medications were centrally stored in medication carts located in the hallway and were inaccessible to residents. Medications were checked for labels, expiration dates, and were properly documented on the centrally stored medication and destruction record. No errors were observed during the medication review.

At 10:04AM, the LPA and Staff conducted a brief tour to ensure the health and safety of the residents in care, and no immediate concerns were observed.

No deficiency cited. Exit interview conducted. A copy of the report was reviewed and provided.
