Facility Evaluation Report
Licensing Program Analyst (LPA) Cynthia Chan conducted the required annual inspection. LPA arrived unannounced and met with Staff. The reason for the visit was explained. Administrator, Angela Porter, arrived shortly thereafter to assist with the visit. The facility is licensed for (4) adults, ages 18 - 59, of which 1 may be non-ambulatory. The facility is vendorized by the San Gabriel/Pomona Regional Center.
The facility consists of 4 client bedrooms, 2 bathrooms, living room, dining room, kitchen, recreation room, and attached garage. The laundry machines are located in the garage. The outdoor has a shaded area with table and chairs. Each client has their own bedroom with appropriate furniture and storage space. There are extra linens and hygiene supplies. Knives, cleaning solutions, and disinfectants are locked. The hot water temperature was measured between the required range of 105-120 degrees F. Staff are providing care and supervision to meet the clients' needs and assisting in activities of daily living. Staff are continuing to follow their infection control plan and procedures while handling clients. There are sufficient food supplies of 2 day perishable and a week of non-perishable items. LPA reviewed records for 3 personnel. The administrator certificate expires on 12/7/26. Staff have current CPR & First Aid certificates. Staff do not use any manual restraints on clients. LPA reviewed 4 client files and they have the required documents. Medications are centrally stored and inaccessible to clients. The facility has the updated emergency and disaster plan and drills are conducted at least quarterly for each shift.
No deficiencies were issued today. An exit interview was held and a copy of this report along with appeal rights was given to the administrator.
