Facility Evaluation Report
At approximately 9:45 a.m. on 03/05/26 Licensing Program Analyst (LPA) Nicholas Reed conducted an unannounced case management visit in conjunction with complaint visit for investigation #31-AS-20260227093402. LPA met with staff and disclosed the reason for the visit.
During the complaint visit, LPA observed Staff #1 (S1) in the facility providing care and supervision to residents. S1 was not associated to the facility. Interview with the licensee at 1:45 p.m. today revealed S1 was not associated to the facility but had a criminal background clearance. The licensee stated S1 had worked in the home for one (01) month. At 2:00 p.m., LPA conducted a file review and did not find S1’s criminal background clearance. A deficiency is issued for S1 working in the facility for more than five (05) days without a criminal background clearance. A civil penalty in the amount of $500 is hereby assessed.
