Facility Evaluation Report

Licensing Program Analyst (LPA) Yelena Avetisyan a conducted a Case Management Deficiencies visit to address deficiencies observed during the investigation of complaint control 31-AS-20210604142033 .

During the course of the investigation information obtained revealed that Staff 1 (S1) has been working at this facility since early 2019. S1's criminal record clearance was not transferred to this facility until 11/9/2021.

During todays visit while speaking with Staff and reviewing records LPA observed that R1 and R2 depend on others to perform all activities of daily living for them . R1 and R2 are not currently receiving hospice care and the licensee/administrator did not submit an exception request.

Pursuant to CCR, Title 22, Division 6, Chapter 8, the following deficiencies are cited (Refer to LIC 809-D).
