The inspector’s account

Licensing Program Analyst (LPA), Angela Panushkina, conducted a CASE MANAGEMENT visit and met with an Executive Director. LPA explained the reason for the visit.

During the visit, made on 09/01/22, LPA was informed that several incidents took place between 08/03/2022 to 08/22/2022 and were not submitted to the Community Care Licensing Department (CCLD) in a timely manner. Based on Title 22 Regulation: a written Unusual Incident / Injury Report shall be submitted to CCLD within seven (7) days of occurrence. LPA informed Executive Director that all staff members are mandated reporters and they are all responsible for reporting.

On 09/02/22, LPA received twelve (12) incidents that occurred on following dates:

Per the California Code of Regulations, Title 22, Division 6, Chapter 8, deficiencies are cited and noted on LIC 809D.

Exit interview conducted, appeal rights and copy of report given emailed to the Executive Director.
