The inspector’s account

On 06/15/26, the department conducted an unannounced annual required visit using the CARE Inspection Tool. The department met with Caregiver, Iris Baclia-An, and the purpose of today’s visit was explained. The department was granted entry to the facility. Administrator Charesa Reyes, joined the department for the visit shortly after. The facility is licensed to operate for six (6) ambulatory of which (2) may be bedridden elderly adults ages 60 and above. The facility has an approved hospice waiver for (4) hospice residents. There are currently (6) residents residing in the home.

The facility is a single-story structure located in a residential neighborhood. It consists of the following: four (4) resident bedrooms, two (2) bathrooms, living room, dining room, kitchen, and an outside shaded patio area.

The department toured the physical plant. There were no bodies of water or obstructions on the premises. All rooms were inspected. Beds and bedding supplies were in good condition, adequate lighting was provided, and storage for the resident's personal belongings was observed. Bed linens, comforters, and bath towels were adequately stocked at the time of the visit. Bathrooms were found to be within Title 22 regulations and were clean and operational. The water temperature measured at 109.0 F. A comfortable temperature was maintained in the facility.

The department observed the facility to be sanitary and appropriately furnished during the visit. Storage areas for personal hygiene, cleaning supplies, toxins, and sharps objects were stored and not accessible to residents.
