Facility Evaluation Report

On 11/05/25, Licensing Program Analyst (LPA) Regina Cloyd conducted an unannounced Required – 1 Year Inspection to the above-named facility and met with Program Director Rachel Ramirez. LPA explained the purpose of the visit and was accompanied by Program Director inside and outside the facility during this inspection.

The facility is a single-story structure that consists of the following: Leisure Room, Art Room, Staff Break Room, Changing Room, Program Director and Case Management Office, Quiet Room, Locker Room, Computer Room, Style Room, Fitness Room, Supply Room, and Kitchen.

The facility is licensed to serve eighteen (18) developmentally disable adults ages 18 and over. Six (6) may be non-ambulatory. There are currently four clients who are non-ambulatory. The clients are Harbor Regional Center consumers. Annual fees are current.
