Facility Evaluation Report
Licensing Program Analyst (LPA), Mayra Cota, conducted an unannounced annual visit today. LPA met with Ashley Crutchfield, Administrator, and the reason for the visit was explained.
The facility is licensed to serve (5) non-ambulatory, developmentally disabled adults ages 18 through 59; (5) may be bedridden and there is a Hospice waiver for (2). The facility is operating within the scope of its license. Currently, there are (5) clients living in the home. Clients are provided services through the Harbor Regional Center.
The single-story home is in a residential area of Torrance and consists of the following (5) client bedrooms, (2) bathrooms, living room, dining area, laundry/medication room, attached garage and front and backyard and storage room.
The following was observed during today’s visit:
Facility is clean and walkways and exits are free of debris and obstructions. Furniture throughout the facility is in good repair. Bedrooms have the required furniture such as beds frames dressers, lamps, and chairs. Clients’ beds are clean and mattresses have the appropriate pads and bedding. Laundry room is also clean and washer and dryer were observed to be operational. Sufficient seating was observed in the living room. Activity items are also available in the garage and are readily available for use.
