Facility Evaluation Report

On 05/29/2025 at 1:20pm, Licensing Program Analyst (LPA) Zina Brown conducted an unannounced annual required visit using the CARE Inspection Tool. At LPA met with the Ashley Crutchfield, Administrator and the purpose of today’s visit was explained. The facility is licensed to operate for age range 18 through 59 for (5) non-ambulatory of which 5 may be bedridden with an hospice waiver approved for two (2). Currently, the home has (4) clients. The clients are Harbor Regional Center clients. None of the clients have Restricted Health Care Conditions and none are utilizing postural supports or protective devices.

The facility has a active administrator certificate (#6073585735) for Ashley Crutchfield valid 12/08/2024 - 12/17/2026 The facility fees are current The facility has liability insurance with Philadelphia Indemnity Ins Co (NAIC#18058) with each occurrence at $1,000,000 and general aggregate at $3,000,000 (policy #PHPKG2624399-014) effective date 10/06/2024 - 10/06/2025. The facility fees is a balance of $454 due on 06/21/2025. LPA provided the facility with the pin # 720707 the option to submit payment online for annual fees.

The facility is a single story home located in a residential neighborhood. The property consists of the following: three (5) bedrooms, two (2) bathrooms, a kitchen, a dining room, a medication home with a washer and dryer an attached garage with an additional refrigerator and an outdoor shaded area in the backyard.
