Facility Evaluation Report

On 09/08/2025 at 11:55 am, Licensing Program Analyst (LPA) Zina Brown conducted an unannounced annual required visit using the CARE Inspection Tool. At LPA met with the Clare Mills-Grey , Administrator and the purpose of today’s visit was explained.

The facility is licensed to operate for 4 non-ambulatory developmentally disable adults with a age range of 18 - 59 with rooms 1, 2,3, and 4 designated for non-ambulatory and a Hospice Waiver for 2. Currently, the home has (4) clients. The clients are Harbor Regional Center clients. None of the client have Restricted Health Care Condition nor are not utilizing postural supports and protective devices.

The facility has a current administrator certificate (# 7007961735 ) for Pamela Broderick (Executive Director) valid from 03/31/2025 - 03/30/2027 and (#6074477735) for Clare J Mills-Grey valid 01/18/2025 - 01/17/2027. The facility has liability insurance with Philadelphia Idemnity Ins (NAIC# 18058) with each occurrence at $1,000,000 and general aggregate at $3,000,000 (policy #PHPKG2624399-014) effective date 01/03/2025 - 01/03/2026. The facility fees are current.

The facility is a single story home located in a residential neighborhood. The property consists of the following: four (4) bedrooms, two (2) bathrooms, one (1) activity room, one (1) office room, a kitchen, a dining room, a living room, an attached garage with a double washer and dryer, a storage of cleaning supplies(locked up), additional can goods and an outdoor shaded area in the backyard.
