On 09/18/2025, Licensing Program Analyst (LPA) Regina Cloyd conducted a case management - health check visit. LPA met with House Manager Evelyn Allego. The facility is licensed for six residents and approved for two hospice residents.
LPA toured the facility and three out of four residents were present, alert, and greeted LPA Cloyd.
An exit interview was conducted and a copy of this report was provided to the House Manager Evelyn Allego.
