On 04/24/2025, Licensing Program Analyst (LPA) Socorro Leandro conducted an unannounced annual inspection to the above facility. LPA met with the Licensee/Administrator, Antonio Arboleda and the purpose of the visit was explained.
A tour of the facility was conducted and records were reviewed.
Due to time constraints the annual inspection was not completed.
An exit interview was conducted and a copy of this report was left with the Licensee/Administrator, Antonio Arboleda.
