The inspector’s account

On 02/13/2026, Licensing Program Analyst (LPA), Wendy Gibbs, conducted an unannounced Required 1-Year Annual Visit to the facility listed above. LPA met with Assistant Administrator, Mariel Ventura, and were later joined by Administrator Hai Nguyen and the purpose of today’s visit was explained. LPA was granted entry into the facility.

The facility is licensed to serve five (5) clients, four (4) ambulatory, and one (1) non-ambulatory between the ages of 15 and 59, with an approved Hospice Waiver for two (2) clients.

Physical Plant/Structure The facility is a single-story structure in a residential neighborhood. It consists of four (4) bedrooms, Staff Office, three (3) bathrooms, living area, activity area, dining area, kitchen, attached garage, and outside shaded patio/activity area. LPA observed all walkways outside the facility are clean, clear, and free of debris, hazards, and obstructions. LPA observed the shaded patio has a table and chairs available for Client use. LPA did not observe any bodies of water on the premises.

Bedrooms LPA inspected the Client bedrooms and observed them to be clean and in good repair. LPA observed Client bedrooms have the required furniture including a bed, dresser, nightstand, chair, and ample storage space for Client personal belongings. LPA observed beds have the required linens including a mattress cover, fitted sheets, blanket, comforter, and pillows. LPA observed an ample supply of linens in a closet in the activity area. All bedrooms were observed with ample lighting.

Bathrooms LPA inspected the three (3) bathrooms and observed them operable and in good repair. LPA observed the showers have non-skid mats and are free of mold and mildew. LPA observed the showers and toilets have secured safety handrails. LPA observed an ample supply of toiletries, towels, and wash clothes in a closet in the hallway. The water temperature measured 109.0-degrees, 110.3-degrees, 109.2-degrees Fahrenheit.
