Facility Evaluation Report
On 06/03/2026 at 2:00pm, the Department conducted an unannounced Required – 1 Year Inspection to the above-named facility and met with the Administrator Lisa Seibel. The Department explained the purpose of the visit. This facility is licensed to serve 4 ambulatory, of which 3 maybe non-ambulatory from ages 18 - 59. All 4 clients are with Harbor Regional Center.
The facility has a current administrator certificate for Lisa Seibel #7033462735 valid 04/21/2025 - 04/20/2027 for ARF.
The facility fee were due on 05/24/2026 which are a balance of $681. The Department provided the facility with the PIN #119358 with the option to submit payment online.
The facility is a one-story house located in a residential neighborhood. It consists of four (4) bedrooms, two (2) bathrooms, a garage, a living room, family room, dining area, and kitchen.
Between the hours of 2:30pm - 2:45pm, the Department conducted a records review of (4) client records, (4) staff records, (4) clients Personal & Incidental Records, (4) Client Medication Administration Records and did not observe any discrepancies at the time of visit. Also the Department reviewed the facility disaster plan. All client & staff records were complete. The facility disaster plan was current and in compliance with Title 22 at the time of visit. Last earthquake/fire drill conducted on 02/10/2026 (earthquake) & 03/25/2026 (fire)
Between the hours of 2:08pm - 2:28pm, the Department & Lisa Seibel (Administrator) toured the inside and outside of the facility. All client rooms were checked. Mattresses and box springs were in good condition, adequate lighting was observed, plenty of dresser and closet space was observed. Bed linens, comforters and bath towels were adequately stocked at the time of visit. Bathrooms were found to be within Title 22 regulation. Toilets and water faucets worked properly . The shower was free of mold/mildew, there is adequate lighting, and sufficient toiletries accessible to clients. The water temperature properly measured between 105F°-120F° degrees (Kitchen 120.F° & Bathroom #1 117.6F° )
