Facility Evaluation Report
On 09/25/2025, Licensing Program Analyst (LPA) Regina Cloyd conducted an unannounced case management - incident visit. LPA met with Administrator Lisa Seibel and explained the purpose of the visit. The Regional Office received multiple unusual incident reports regarding Client #1's behavior at the facility. During today's visit, LPA observed Client #1, reviewed Client #1's record, interviewed Staff #1-4, and attempted to interview Client #2. Clients #3-4 were still at Day Program.
Deficiencies are being cited according to Title 22, see LIC809-D.
Record review of Client #1's file does not include a medical assessment, LIC602. C1 was admitted to the facility on 08/20/2025.
An exit interview was conducted, plans of correction developed, and a copy of this report and appeal rights was provided to the Administrator Lisa Seibel.
