The inspector’s account

On 11/19/2025, at 10:00am, an office meeting was held to discuss Complaint 11-AS- 20230117153703. Present at the meeting were Eva Alvarez, Licensing Program Manager (LPM), Wendy Gibbs, Licensing Program Analyst (LPA), Judith Uy-Villaruz, Executive Director, Jennifer Larsen, Regional Health Services Director, Jill Libhart, Vice President of Operations , and Jen Sato, Senior Vice President of Health Services.

During the meeting, the LPM reviewed the details of the Complaint. On January 27, 2024, the Department substantiated an allegation of Resident wandered away from the facility due to lack of supervision resulting in hypothermia

At this time the Department is considering an enhanced civil penalty, pursuant to Health and Safety Code Section 1569.49(f).

The Department is reviewing the complaint for an enhanced civil penalty for serious bodily injury pursuant to H&S 1569.49(f). The total amount for the civil penalty totals $10,000 for Serious Bodily Injury.

An exit interview was conducted with Judith Uy-Villaruz, Executive Director, and a copy of this report was provided.
