The inspector’s account

On 03/10/2026, Licensing Program Analyst (LPA), Wendy Gibbs, conducted an unannounced annual visit to the facility listed above. LPA met with Program Director, Daniel Morales, and the purpose of today’s visit was explained. LPA was granted entry into the facility.

The facility is licensed to serve developmentally disabled client’s aged 18 and over. The facility is approved for 21 ambulatory clients, 6 of which may be non-ambulatory. During the visit there were ten (10) clients present and six (6) in the community.

Physical Plant/Structure The facility is a single-story building in a commercial area. It consists of a reception area, staff offices, 2 conference rooms, supply/copy room, employee break room, fitness room, leisure room, computer room, arts and crafts room, salon, quiet room, 3 storage rooms, kitchen, music rooms, 4 bathrooms, and locker room. LPA observed all walkways, hallways, and doorways were clean, clear, and free of obstructions, hazards, and debris. LPA did not observe any bodies of water on the premises.

Rooms LPA inspected all activity rooms and found them to be clean and in good repair. All rooms were observed appropriately furnished during time of visit. All rooms were observed with ample lighting. The facility was maintained at a comfortable temperature. LPA observed an ample supply of reading material, activities, crafts, games, exercise equipment and other recreational materials for clients use.

Bathroom LPA inspected the Men’s, Women’s, and two (2) unisex restrooms and found they are operational, clean, and sanitary. All bathrooms have secured safety handrails. The water temperature in the bathrooms measured 106.4-degrees, 108.3-degrees, 111.7-degrees, and 114.8-degrees Fahrenheit. LPA observed an ample supply of hand soap and paper towels secured in a storage room.

Kitchen LPA observed the kitchen is clean and sanitary. All appliances were observed in good working repair. LPA observed an ample supply of cookware, dishware, and cutlery. LPA observed a 7-day supply of
