Facility Evaluation Report
On 05/23/2025, Licensing Program Analyst (LPA), Wendy Gibbs, conducted an unannounced Annual Visit to the facility listed above. LPA met with Administrator, Bienvenido ‘Bien’ Cadungog, and the purpose of today’s visit was explained. LPA was granted entry into the facility. The facility is licensed to serve six (6) ambulatory and non-ambulatory residents with an approved hospice waiver for one (1).
Physical Plant/Structure The facility is a single-story house in a residential neighborhood. It consists of the following: six (6) resident rooms, three (3) bathrooms, sitting area, living room, kitchen, dining area, laundry area, detached garage, front yard, and shaded back patio. LPA observed all walkways around the facility are clean, clear, and free of obstructions, debris, and hazards. LPA did not observe any bodies of water on the premises.
Bedrooms LPA inspected all resident bedrooms and observed them to be clean and in good repair. LPA observed all rooms have the required furniture including a bed, dresser, nightstand, chair, storage space for resident’s belongings, and ample lighting.
