Facility Evaluation Report

On 04/09/2026 at 8:45am, the California Department of Social Services (CDSS) Community Care Licensing Division (CCLD) conducted an unannounced required – 1 year annual inspection to the above-named facility and met with Administrator, Dwain Griffith. The Department explained the purpose of the visit and was allowed entry to the facility. This facility is licensed to serve 6 ambulatory adults ages 18 to 59 years old. Its a total of 4 clients who are Harbor Regional Center clients currently residing at this facility.

The facility has a current administrator certificate for Dwain Griffith (#7020227735) valid 10/31/2025 - 10/30/2027. The facility fees are current. The facility fees a balance of $454.00 due on 03/28/2026. LPA provided the facility with the PIN # 928383 with the option to make online payment.

The facility is a one-story house located in a residential are. The home consists of: 4 client bedrooms; 2 full bathrooms; 1 living room; 1 kitchen area with a dining table and an office area; 1 laundry room in the attached garage with a gym area, an additional refrigerator and cabinet full of canned good; backyard with shaded area and there is a underground pool that was filled and now there is a small lawn area there.

Between the hours of 9:05 AM - 10:35am, the department conducted a records review of (4) client records, (6) staff records, (3) Client Medication Administration Records, (3) clients Personal & Incidental Records, and the facility disaster plan. The facilities last disaster drill was current & conducted on 03/12/2026 and in compliance with Title 22 at the time of visit.
